Arranging the form fields on top of an image

Check out the code I've been working on here.

I'm facing an issue where the text disappears when I try to position it next to an image. Is it possibly going behind the image? Thank you for your assistance and time.

In case there is a need to maintain the image in the forefront, one can consider using z-index:-1; to relegate something to the background. Increasing values will elevate it to the foreground; with the default value being 1.
