Top technique for creating a unique cursor image for a website

I'm looking for the most efficient way to create a custom cursor image for my CSS/HTML5 website without using Flash. Ideally, I would like to implement this using CSS rather than resorting to Javascript. If Javascript is necessary, I want to know which solution requires the fewest libraries while maintaining compatibility with all browsers. Your assistance is greatly appreciated.

If you want to specify the cursor style using CSS, you have the option to define either a type or a URL for the image you wish to utilize:

cursor: url('image.gif'), pointer; 

Note that IE6 only supports images in .cur or .ani formats.
